1. Aged 18 and older
2. Pregnant at any point in gestation (with singleton or multiple gestation)
3. History of congenital and/or acquired heart disease defined as the following:
4. valvular, congenital, ischemic heart disease or cardiomyopathy,
5. clinically significant maternal arrhythmias in women,
6. current or previous history of peripartum cardiomyopathy,
7. supraventricular tachycardia,
8. placement of either a pacemaker or electrical assist device,
9. aortopathies (Marfan syndrome, Loey's Deitz, Ehlers Danlos \[vascular subtype\],
10. pre-pregnancy diagnosis of pulmonary hypertension
11. English- or Spanish-speaking
