• Social Support Network Counseling Intervention (BEHAVIORAL)

1. Determined by transplant center to be suitable for continuation in evaluation for listing (or a member of that person's social support network)
2. 18 Years of age or older
3. English-speaking
4. Willingness to participate with at least 2 social network members in the session and have calculator results shown to social network members (if a patient)
5. Able to provide informed consent Exclusion
6. Prisoners
7. Individuals with impaired ability to consent
8. Pregnant females.
9. Patients who choose to opt out of research.
10. Non-English speaking
